Produce a one-page checklist template (in English and a locally relevant language) with fields for outage type, date, time, duration, location, and impact. Pilot with 10 households in one neighborhood for four weeks, coordinated by a local community leader who submits the forms to the municipal office.

Hypothesis: A standardized, paper-based checklist increases the accuracy and frequency of outage reports compared to ad hoc or digital-only methods, especially in low-connectivity areas. Proposed validation: Proposed pilot target: At least 80% of participating households submit completed checklists each week for 4 weeks. Baseline: number of outage reports received by the municipality before the pilot. Success measured by increased reporting rate and completeness of data.
